Our organization operates Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Providing ongoing skilled support and compassionate daily assistance for residents.
- Short-term, sub-acute rehabilitation: Assisting patients in their recovery from surgery, illness, or injury.
- Collaborative care: Utilizing an interdisciplinary team approach where nursing, therapy, and medical staff work in unison.
The patient demographics may vary slightly across different facilities. As a therapy professional, your role will be to support patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused environment.
Position Overview
The Physical Therapist (PT) is responsible for delivering direct rehabilitation care in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Qualifications
- Graduation from an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT).
- Preferred: At least one (1) year of experience in a post-acute care or similar setting.
- Active participation in community health initiatives and projects as appropriate.
- Proficiency in Microsoft Word, Excel, and email communication.
- Ability to liaise effectively with patients, families, and support departments.
- Knowledge of practices, procedures, and regulations governing operations in post-acute care facilities.
Key Responsibilities
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
- Maintain confidentiality regarding all proprietary and confidential information.
- Understand and adhere to organizational policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing all mandatory compliance training.
- Follow established physical therapy treatment plans for patients.
- Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
- Utilize therapy software accurately and effectively.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Read, write, speak, and comprehend the English language proficiently.
