Job Overview
This position is a full-time role with PRN opportunities. Our organization operates Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Providing ongoing skilled support and compassionate daily assistance to residents.
- Short-term, sub-acute rehabilitation: Assisting patients in their recovery from surgery, illness, or injury.
- Collaborative care: Engaging in an interdisciplinary approach where nursing, therapy, and medical staff work in unison.
The patient demographics may vary slightly across different facilities. As a therapy professional, your role will be to support patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused environment.
Key Responsibilities
The Physical Therapist (PT) will:
- Deliver direct rehabilitation care in physical therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.
- Participate in community health initiatives and projects as appropriate.
- Maintain effective communication with patients, families, and support departments.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
- Ensure confidentiality of all proprietary and sensitive information.
- Comply with organizational policies, including those related to harassment and compliance procedures.
- Exhibit integrity and professionalism by adhering to the organization’s Code of Conduct and completing all required compliance training.
- Follow established physical therapy treatment plans for patients.
- Supervise and assess the care provided by Physical Therapist Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
- Utilize therapy software accurately and effectively.
- Provide excellent customer service and maintain a positive attitude towards patients.
Qualifications
- Must be a graduate of an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT).
- Preferred: At least one year of experience in a post-acute care or similar setting.
- Proficient in Microsoft Word, Excel, and email communication.
- Ability to read, write, speak, and understand the English language.
