About Our Facilities

Our organization operates as Skilled Nursing Facilities (SNFs), offering:

  • Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.

Position Overview: Physical Therapist (PT)

The Physical Therapist provides direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications
  • Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT). Graduates from 2003 onwards must possess either an MSPT or DPT.
  • New graduates are welcome to submit their applications.
Responsibilities
  • Engage in community health initiatives and projects as appropriate.
  • Demonstrate proficiency in Microsoft Word, Excel, and email communication.
  • Act as a liaison with patients, families, and support departments.
  • Maintain knowledge of practices, procedures, and regulations governing operations in post-acute care settings.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Ensure confidentiality of all proprietary and sensitive information.
  • Adhere to company policies, including those related to harassment and compliance.
  • Demonstrate integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
  • Follow established physical therapy treatment plans for patients.
  • Supervise and assess the care provided by Physical Therapy Assistants (PTAs), Rehabilitation Aides, and PT and PTA students.
  • Utilize therapy software accurately and effectively.
  • Provide excellent customer service and maintain a positive attitude towards patients.
  • Read, write, speak, and comprehend the English language proficiently.
