Full-Time Advanced Practice Nurse

The APN license is provided by NJ Division of Consumer Affairs to Master's prepared RN's meeting criteria as either Clinical Nurse Specialists or Nurse Practitioners. Nurses licensed as APN's who also have attained Prescriptive Authority and DEA registration may provide medical services in conjunction with a licensed physician or group. At Center for Hope Hospice, the APN may provide medical visits to patients under the agency's Medical Directors and may provide the medical evaluation and documentation of a Face-to-Face visit to satisfy the CMS requirement for this site visit for those Hospice beneficiaries entering any '60-day' benefit period. Such visits include:

• Review of available clinical documentation for the patient;
• Consultations with the primary RN, IDG members, Hospice Physician and Attending Physician (if different), before &/or after the in-person assessment of the patient;
• Direct assessment of the patient and documentation of findings, including patient/family reports of changes, etc., as well as appropriate coding of visits for reimbursement as a medical visit, where and when appropriate, as for Palliative Care patients not covered by the Hospice benefit;
• Recommendation for re-certification, or discharge, to the Hospice Physician; and/or
• Assessments of Hospice &/or Palliative Care patients as directed by the Hospice Physician; and,
• Completion and submission of all documentation required by law, regulations, professional standards and the Center's policies.

Note that while the APN may accept and perform any duties appropriate for a skilled Registered Nurse for Center for Hope Hospice; in this role, the APN's function is maintained as distinct from such other skilled nursing functions she or he may also perform for the Center.
