This onsite role gives you the chance to work directly with individuals in need while using your empathy, problem-solving skills, and commitment to helping others grow. This position offers competitive pay ranging from $21.15–$29.58/hour, along with a supportive team culture that values innovation, compassion, and excellence in mental health care.

Schedule: Monday–Friday, 8:00 AM–5:00 PM
Compensation: $21.15–$29.58 DOE and Licensure

What You'll Do as a Case Manager
- Conduct client assessments and gather essential background information
- Develop individualized care plans based on client needs
- Provide ongoing support, guidance, and resources to help clients achieve their goals
- Coordinate with clinicians, therapists, and other healthcare providers
- Maintain accurate documentation and case notes
- Build strong rapport with clients while offering consistent, compassionate care
- Collaborate with a dedicated onsite team to support both clients and clinicians
Qualifications
Applicants must meet ONE of the following:
Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.
- Associate's degree or equivalent and working toward SSW licensure
- Bachelor's degree with Social Service Worker (SSW) license
- Bachelor's degree in any field with willingness to pursue SSW licensure
Additional Requirements:
- Valid driver's license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Spanish speakers may qualify for increased wages after testing
