We value collaboration, innovation, empathy, and creating a workplace where both clients and employees can thrive.What You'll Do as a Case ManagerConduct client assessments and gather essential background information.Develop individualized care plans based on client needs and treatment goals.Provide ongoing support, guidance, advocacy, and resources to help clients achieve greater stability and independence.Help clients access community resources, housing assistance, benefits, and healthcare services.Assist with daily living needs, including coordinating medical, dental, and vision appointments.Coordinate care with clinicians, therapists, nurses, prescribers, and other healthcare providers.Build strong rapport with clients while providing consistent, compassionate support.Maintain accurate documentation, case notes, and client records.Collaborate with the onsite team and community partners to support clients and clinicians.QualificationsApplicants must meet ONE of the following:Note: Applicants under the "working toward SSW" pathway must obtain their SSW license within 24 months of hire.Associate's degree or equivalent and working toward SSW licensureBachelor's degree with Social Service Worker (SSW) licenseBachelor's degree in any field with willingness to pursue SSW licensureAdditional Requirements:Valid driver's licenseBasic understanding of Social Work principlesFamiliarity with social welfare policy and human developmentAbility to problem-solve and maintain a client-centered approachSpanish speakers may qualify for increased wages after testingApply TodayReady to make a real impact?
