Home Health Physical Therapist (PT) Job Responsibilities

  • Perform comprehensive patient evaluations and develop individualized treatment plans
  • Conduct start of care assessments and complete OASIS documentation accurately
  • Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence
  • Complete patient recertifications and reassessments as required
  • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
  • Monitor patient progress and modify treatment plans to maximize outcomes
  • Maintain timely and accurate clinical documentation using Matrix EMR
  • Deliver compassionate, patient-centered care in a home-based setting

Education & Certification Requirements

  • Doctorate or Master’s degree in Physical Therapy from an accredited program
  • Active Missouri Physical Therapist (PT) license required
  • Current CPR/BLS certification preferred
  • Home health experience is not required
  • Strong communication, organization, and time management skills
  • Ability to work independently and manage a daily patient schedule
  • Experience with OASIS documentation and Matrix EMR preferred but trainable
