Connected Health Care is partnering with a respected healthcare organization to recruit an experienced Hospice RN Case Manager for a full-time opportunity in Boise, Idaho. The Hospice RN Case Manager provides skilled nursing care in the home and community setting while coordinating an interdisciplinary hospice team. This role requires strong clinical assessment skills, compassionate communication, effective care coordination, and the ability to work independently while supporting patients and families through complex healthcare needs.
Job Description
Provide compassionate, skilled nursing care to hospice patients in their homes and other appropriate care settings.
Serve as the primary Hospice RN Case Manager for an assigned patient caseload.
Perform comprehensive nursing assessments and evaluate patients' physical, emotional, psychosocial, and spiritual needs.
Develop, implement, and regularly update individualized hospice plans of care.
Coordinate patient care with physicians, hospice aides, social workers, chaplains, volunteers, and other interdisciplinary team members.
Monitor changes in patient condition and adjust the plan of care as appropriate.
Communicate significant changes in patient status to the attending physician and hospice medical director.
Provide education and emotional support to patients and family members regarding disease progression, symptom management, medications, and end-of-life care.
Manage pain and other symptoms while promoting patient comfort and dignity.
Administer medications and treatments in accordance with physician orders and the patient's individualized plan of care.
Evaluate the effectiveness of interventions and make appropriate recommendations regarding patient care.
Assist patients and families in understanding hospice services and the goals of comfort-focused care.
Coordinate care transitions and communicate effectively with other healthcare providers when necessary.
Maintain accurate, timely, and thorough clinical documentation in accordance with organizational, state, and federal requirements.
Participate in interdisciplinary group meetings and hospice care planning activities.
Collaborate with the hospice team to ensure continuity and quality of care.
Provide appropriate end-of-life education and support to family caregivers.
Respond appropriately to urgent patient and family needs within the scope of hospice services.
Follow applicable hospice regulations, policies, procedures, and standards of nursing practice.
Maintain patient confidentiality and protect sensitive healthcare information.
Manage a field-based schedule efficiently while traveling between patient visits throughout the Boise area.
Education and Certification Requirements
Current and unrestricted Registered Nurse RN license in Idaho or eligibility for Idaho RN licensure.
Associate Degree in Nursing ADN from an accredited nursing program required; Bachelor of Science in Nursing BSN preferred.
Current BLS/CPR certification for healthcare providers.
Previous hospice, palliative care, home health, or related nursing experience preferred.
Valid driver's license and reliable transportation required for home and community-based patient visits.
Ability to meet applicable automobile insurance requirements.
Strong clinical assessment, documentation, communication, and organizational skills.
Preferred Qualifications
Previous experience as a Hospice RN Case Manager.
Hospice or palliative care nursing experience.
Home health nursing experience.
Familiarity with Medicare hospice regulations and requirements.
Knowledge of pain management and symptom control.
Experience working with interdisciplinary healthcare teams.
Strong understanding of end-of-life care and family support.
Ability to work independently while maintaining effective communication with the hospice team.
Compassionate, patient-centered approach to care.
Strong time-management and case-management skills.
