Position Summary
Louisiana Community Care is seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) to join our team in our Intermediate Care Facility for Individuals with Intellectual and Developmental Disabilities (ICFDD) community homes in Ball, LA. In this vital role, you will focus on safe medication administration, medication inventory management, med counts, and quality control processes to support the health and well-being of our residents. This full-time position reports directly to the Director of Nursing and plays a key role in regulatory compliance and resident-centered care.
Key Responsibilities
Accurately prepare, administer, and document medications according to physician orders and facility policies
Perform routine medication counts and maintain accurate medication administration records (MARs)
Conduct quality control checks on medications, supplies, and storage areas to ensure compliance with state and federal ICFDD regulations
Monitor residents for medication effectiveness and potential side effects, reporting any concerns promptly to the DON or physician
Assist with medication ordering, receiving, and reconciliation processes
Maintain organized and secure medication carts and storage areas
Participate in interdisciplinary team meetings regarding resident care and medication management
Ensure all documentation meets regulatory standards
Support a safe, clean, and therapeutic environment for residents with intellectual and developmental disabilities
Follow infection control, safety, and agency protocols at all times
Qualifications & Requirements
Current, unencumbered LPN license in the state of Louisiana
Minimum 1 year of nursing experience (long-term care, ICFDD, developmental disabilities, or similar setting preferred)
Strong attention to detail with excellent organizational and documentation skills
Basic computer skills and familiarity with electronic medication records
Current CPR/BLS certification
Ability to work effectively as part of a team and communicate clearly with residents, families, and staff
Must pass background check and drug screening
