Synergy Care is hiring a full-time Occupational Therapist for an AL/IL/Memory Care community-Bethesda Gardens in Fort Worth, TX!

We are looking for a team-oriented clinician who is passionate about providing quality patient care and making a difference in the lives of others.

• Job Responsibilities:• Evaluate patients and develop individualized treatment plans• Provide ongoing treatment and discharge planning• Supervise assistants and aides• Participate in case management• Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines• Represent Synergy Care’s commitment to excellence in every patient and facility interaction
