They currently boast a team of more than 50 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.POSITION DESCRIPTION: The physician serves as the primary care provider for a dedicated panel of geriatric patients.Patients are located in long-term care facilities (ALFs / ILFs) and private homes. Physicians are provided a convenient geographic area in which all patients are located and there is flexibility in regard to whether house call patients are included in the panel.Physicians see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs.
