Full Time Registered Nurse – Home Health Admissions 
Location:Overland Park, KS with occasional coverage in Missouri

This position will primarily serve patients in Overland Park, Kansas, with occasional coverage in Missouri

Position Overview
The Registered Nurse will focus on completing home health admissions and start-of-care visits, conducting comprehensive patient assessments, and ensuring accurate OASIS documentation. The RN will work closely with interdisciplinary care teams to develop care plans and ensure patients receive appropriate home health services.
Key Responsibilities

• Perform home health admissions and start-of-care visits for new patients
• Conduct comprehensive patient assessments in the home setting
• Complete and document OASIS assessments accurately and in compliance with regulatory standards
• Utilize Matrix EMR for patient documentation and care planning
• Collaborate with interdisciplinary care teams to develop individualized care plans
• Educate patients and families on treatment plans, medications, and home health services
• Ensure all documentation meets state, federal, and agency compliance requirements
• Coordinate care with physicians, therapists, and other healthcare professionals
• Monitor patient conditions and communicate changes to appropriate clinical staff
• Travel between service areas as needed, primarily in Independence with occasional coverage in Overland Park

Education and Certification Requirements

• Active Registered Nurse license required
• Must hold a Compact RN License or active licenses in Kansas and Missouri
• Graduate of an accredited nursing program (ADN or BSN)
• Current Basic Life Support certification preferred
• Strong knowledge of home health regulations and documentation standards

Experience Requirements

• Minimum 1 year of nursing experience required
• Home health experience required
• OASIS documentation experience required
• Experience performing admissions or start-of-care visits strongly preferred
• Experience using Matrix EMR preferred
• Strong clinical assessment and communication skills
