Our team specializes in providing evaluation and treatment for feeding difficulties related to a variety of factors including medical, psychosocial, sensory, and skill-based needs. Speech-Language Pathologist $Do you have experience in speech-language pathology and a passion for working with children and families in a pediatric outpatient setting? We are seeking a Speech-Language Pathologist who is committed to supporting communication development through engaging, individualized, and family-centered care. In this role, you will play an essential part in helping children reach their full potential by completing comprehensive evaluations, assessing speech, language, and feeding/swallowing skills, and developing personalized, evidence-based treatment plans. You will collaborate closely with caregivers to provide education, coaching, and home programming, ensuring that skills generalize to the child’s natural environment. Your clinical expertise and creativity will support meaningful outcomes in areas such as expressive and receptive language, articulation, social communication, and feeding. Direct and administer speech therapy using techniques that assist in the rehabilitation of individuals with functional disabilities of speech, language, voice, and swallowing. Review medical records, test and evaluate individuals served, develop treatment plans, and administer therapy. Provide effective evaluations, documentation, and communication of medical information. Assess speech and language skills, development, articulation ability, and voice and fluency disorders of patients assigned. Develop, implement, and evaluate individualized patient therapy plans, coordinating therapy modifications to the individual’s plan of care with the Director of Clinical Services and the individual’s physician. routinely review pertinent medical data to determine the effectiveness of therapy services in reaching maximum rehabilitation potential for each individual served. Instruct individuals served, family members, and primary caregivers as needed to ensure the maintenance of or acquisition of optimal functioning level for each individual. Ensure effective coordination of home care services through timely completion of required documentation and transfer of pertinent medical data to the individual’s physician, Director of Clinical Services, and other caregivers. Review and document comprehensive evaluation reports to summarize tests utilized, results of test(s), and diagnosis of speech/hearing disorders. Report and record assessments, evaluations, therapy interventions, and patient changes according to physician orders and the Mentor Network’s policies. Investigate and take appropriate actions on all concerns and/or complaints. Master’s Degree in Speech Therapy or Speech/Language Pathology. One year’s experience as a Speech Therapist. Current state licensure/certification as a Speech Therapist.
