Registered Nurse (RN) Supervisor – Full Time + $4,000 SIGN ON BONUS

Transitions Healthcare Washington PA is seeking a dedicated Full-Time RN Supervisor to join our team of talented caregivers.

Schedule: 7:00 PM – 7:00 AM, with rotating weekend coverage required (every other weekend).

Weekend Baylor RN + $4,000 SIGN ON BONUS

Work 32 hours, get paid for 40 — $48 per hour, with overtime available.

Weekend RNs work two 16-hour shifts. If preferred, this schedule can be adapted to two 12-hour shifts, with an additional 8 hours of pay provided to maintain the same total compensation.

Responsibilities

•      Assist in developing individualized care plans for residents requiring advanced levels of preparation and attention.

•      Ensure timely and accurate completion of all documentation requested by the Nursing Office, including monitors and pharmacy reviews.

•      Collaborate with multiple disciplines throughout the facility to maintain high standards of resident care and to communicate updates on resident therapy and progress.

•      Report changes in resident condition promptly to the MDS Coordinator, Supervisor, Restorative Coordinator, and other relevant team members.

•      Prepare clear, thorough summaries documenting residents’ conditions, concerns, and progress to support physicians in delivering appropriate care and coordinating with consulting specialists.

•      Conduct initial assessments for new admissions and residents returning from the hospital; prepare physician order documentation and coordinate medication orders as needed.

•      Perform regular rounds to personally observe and evaluate resident well-being and care conditions.

Qualifications

•      Active Registered Nurse license in good standing with the Commonwealth of Pennsylvania.

•      A minimum of two (2) years of experience in institutional resident care is preferred.

•      Current CPR and AED certification required.
