Full-Time Social Worker - Hospice Albany
Join Our Compassionate Hospice Team the Community Hospice is seeking a full-time Social Worker to join our dedicated interdisciplinary team serving patients and families throughout the Capital Region. This position provides care in patients' homes and nursing facilities, supporting individuals and caregivers as they navigate serious illness and end-of-life care. As a vital member of the hospice team, you will focus on addressing the emotional, psychological, and social needs of patients and families while collaborating closely with nurses, physicians, chaplains, aides, and other healthcare professionals.
What You'll Do:
Provide psychosocial assessments and ongoing emotional support to patients and families. Offer counseling and therapeutic interventions related to end-of-life issues, grief, loss, and adjustment. Identify barriers that may impact patient well-being and quality of life. Connect patients and families with community resources and supportive services. Advocate for patients and help ensure care is provided in the most appropriate setting. Collaborate with the interdisciplinary team to develop and implement patient-centered care plans. Support staff through consultation and education related to psychosocial aspects of hospice care.

Pay Range: $30.00 - $43.08
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
