Job Description:

• Provide comprehensive social work services to patients and their families, including psychosocial assessments, counseling, resource referrals, and discharge planning.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Advocate for patients’ rights and coordinate community resources to ensure continuity of care.
• Maintain accurate and thorough documentation in compliance with regulatory standards and hospital policies.
• Participate in ongoing professional development and training opportunities.

Education and Certification Requirements:

• Master’s degree in Social Work from an accredited institution.
• Current licensure as a Social Worker in the state of Arizona.
• Minimum of 2 years of clinical social work experience preferred.
• BLS certification required.
