Details: Schedule: Fulltime, Monday - Friday position Location: Irvine, CA Call: No after hours call Team: Currently one, fulltime Functional Medicine Physician Staff: Experienced and dedicated support and administrative staff EMR: Epic Position Responsibilities Provide thoughtful, patient-centered primary and functional medicine care focused on understanding each patients unique story and goals Partner with patients to identify lifestyle, environmental, emotional, and biological contributors to their health Develop personalized, attainable care plans that honor each patients readiness, preferences, and capacity for change Approach care with empathy, curiosity, and a commitment to building long-term therapeutic relationships Support patients in managing acute and chronic conditions using a whole-person, systems-based perspective Clearly communicate findings, options, and next steps in a way that builds trust and shared understanding Continuously monitor patient progress and adapt care plans collaboratively Provide preventive care, wellness guidance, and education that empowers patients and supports sustainable change Maintain thorough, accurate documentation of patient encounters, clinical findings, and care plans Refer to specialists appropriately and coordinate care to ensure a seamless patient experience Leads and oversees all aspects of patient care planning, ensuring that treatment recommendations from the Nutritionist and Health Coach/Physiologist are clinically appropriate, aligned, and fully integrated into the patients overall care plan. Maintains ultimate clinical oversight and decision-making authority for each patient, guiding interdisciplinary collaboration and validating all support-staff assessments to ensure cohesive, high-quality care.
