    Description

     At Fortcom Health Services, we are committed to providing quality healthcare that improves patients’ lives in the comfort of their homes. As our organization continues to grow, we are seeking dedicated and detail-oriented Direct Care Aide to join our clinical team.

    Reports To: Program Director

    Need Direct Care Aide to cover Fall River, East Boston

    Qualifications
    • Preferably hold a valid certification as a home health aide
    • One or more years of experience working with elders and people with disabilities providing assistance with ADLs and IADLs
    Responsibilities
    The responsibilities of the registered nurse include:
    1. Provision of hands-on or cueing and supervision with ADLs and IADLs in accordance with the individual’s GAFC plan of care
    2. Provides personal care and related services in the home
    3. Issuing reminders to the member about prescribed medications
    4. Timely refilling of the member’s prescriptions
    5. Completing a direct care aide log for each visit
    6. Sending the completed direct care aide log at the end of each month to the nurse
    7. Monitoring and reporting any nonurgent or nonemergency changes in the member’s medical condition or in the home situation to the staff nurse
    ADLS and IADLS
    • ADLs (Basic Self-Care): Personal hygiene/grooming, dressing, feeding, toileting, and moving around (ambulating/transferring).
    • IADLs (Independent Living Skills): Managing finances, managing transportation, shopping, meal preparation, housekeeping, and managing medications.
    This is a per diem position and will be paid $25 per visit.
    Compensation: $25.00 per day
