HarmonyCares has provided physician-led, home-based primary care for more than 30 years. Operating across 14 states, we bring comprehensive care directly to medically complex patients who often face barriers to accessing a traditional medical office.

Our value-based model gives physicians and APPs the time and support to focus on better outcomes—not the number of visits completed. Providers typically see 5–9 patients per day and collaborate with nurses, social workers, care coordinators, and other clinicians to manage chronic conditions and support patients between visits.

Patient panels are developed through established health plan partnerships. There is no patient acquisition or hospital rounding required.

The Leadership Opportunity

HarmonyCares is building a new home-based primary care practice that will include Dauphin, Lancaster, and Berks counties and is hiring a Physician Lead (Medical Pod Leader) to help establish and guide the clinical team.

The physician selected may be based in any of the three counties. Providers will care for geographically clustered patient panels near their respective communities, while the Physician Lead supports the broader regional team.

This role is designed for a physician who wants to remain clinically active while taking on meaningful leadership responsibilities. Up to 50% of the position may be dedicated to mentoring and supervising Advanced Practice Providers, promoting clinical quality, and helping develop the practice as the market grows. The remaining time will include direct patient care with a smaller personal panel.

Qualifications

• MD or DO with an active Pennsylvania medical license
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Experience collaborating with or supervising Nurse Practitioners or Physician Assistants strongly preferred
• Strong clinical experience caring for medically complex adult patients
• Interest in clinical leadership, value-based care, and relationship-centered medicine
