Augusta Medical Group is seeking a compassionate, relationship-driven physician to join a growing community-based geriatric and transitional care program serving the Shenandoah Valley. This is a growth-focused role with a strong opportunity to help expand services, shape workflows, and build the future of geriatric care across the region.
The position provides care across 7 assisted living facilities and a hospital-based SNF/rehab unit, supporting approximately 234 assisted living patients and ~12 SNF patients (average daily census). The physician will work within a collaborative, supportive team that includes an APP, CMA, administrative support, and strong partnership with local psychiatry services to support a complex behavioral health population.
Key responsibilities include:
- Providing comprehensive primary care to geriatric patients in assisted living and SNF settings
- Managing chronic disease, acute care needs, and preventive care
- Conducting facility-based visits across 1-2 sites per day - less than 20 minutes commuting distance from main location.
- Coordinating care across facilities, hospitals, and specialty providers
- Building strong relationships with patients, families, and facility staff
- Supporting and helping grow the program, including workflow development and quality initiatives
Practice highlights:
- ~10 patients per day in a community-based setting
- Shared home-based call across Transitional Care, Hospice, and Palliative teams
- Weeknights: 6 PM - 6 AM (rotating)
- Weekends: 1:9 rotation

Ideal candidate:
- Board-certified/board-eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Passion for geriatric and underserved populations
- Strong communicator and team player
- Interested in helping grow and develop a rapidly expanding service line
- Experience in assisted living, SNF, or home-based care preferred
Salary
Starting Salary $335,240
Sign-On- $30,000
Relocation: $10,000 (receipt based)
CME: $4200
