SUMMARY OVERVIEW
Under the general direction of the Team Manager, but working collaboratively with the SCO representative(s), the SCO GSSC (Geriatric Support Service Coordinator) plays an integral part of the case management functioning of the agency. The SCO GSSC will provide initial assessment and ongoing assessments of the SCO client's needs including eligibility for any relevant Boston Senior Home Care (BSHC) service and other community resources. Furthermore, the SCO GSSC will develop a service plan based on their assessment of client needs, which directly impacts areas of concern highlighted through the assessment. This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The SCO GSSC is responsible for working collaboratively within BSHC as well as externally with client formal and informal supports, SCO representatives, vendors, potential referral sources and other community agencies. SCO GSSCs actively participate in BSHC organizational life.

TRAVEL
This position required up to 70% travel.
Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.
