Job Requirements

Education Requirements:

• Bachelor’s degree in Occupational Therapy or related field.

Certifications Requirements:

• WI License
• BLS certification
• Valid Driver’s License with active auto insurance

Experience Requirements:

• Minimum of 3 years of occupational therapy experience.
• Previous home health or acute care experience preferred.
• Prior travel therapy experience is a plus.

Other Requirements:

• Ability to support within interdisciplinary teams.
• Strong evaluation and adaptive equipment skills.
• Ability to float to neighboring counties (Kenosha and Racine) as required.

Job Responsibilities

• Collaborate with interdisciplinary teams including RNs and PTs to develop comprehensive care plans.
• Provide occupational therapy evaluations, focusing on ADLs and adaptive equipment.
• Support patients in their home environments to improve functional independence.
• Document assessments, progress, and treatment outcomes accurately within the EPIC EMR system.
• Participate in daily team huddles and communicate effectively across team members to coordinate patient care.
