POSITION SUMMARY:

The Home Health Technician, under the direction of the Eldercare Services Division Director, is responsible for establishing key relations with Sault Tribe elders and families in the home, clinic and community setting. The position is responsible for providing individual assessments, health education and consultation services; therapeutic and follow-up health services under the direction of health care providers. The position is responsible for monitoring and controlling environmental factors affecting health; assuring health and other program utilization; and for resolving physical, economic, environmental, and cross-cultural encumbrances to the attainment of health care.

ESSENTIAL FUNCTIONS: (includes, but is not limited to, the following)

  • Provides direct care services which include the five basic services of the program: Homemaking, Outreach, Chore, Respite Care and Transportation.
  • Facilitates access to health care through advocacy.
  • Checks blood sugars and blood pressures.
  • Medication management and medication delivery.
  • Monitors health conditions of clients.
  • Develops client goal-oriented plans and charting on a timely basis.

ADDITIONAL RESPONSIBILITIES: (includes, but is not limited to, the following)\

  • Informs clients on available social services and aids in completing applications as requested.
  • Provides health education training to clients.
  • Assists with Elder meal program, if needed.

Immediate peers, peers in other departments, immediate supervisor/manager, customers, outside vendors/service providers, families.

PHYSICAL REQUIREMENTS:

Position very heavy with team lifting objects over 100 pounds and frequent lifting/carrying more than 50 pounds. Physical factors include constant standing, walking, lifting, use of hearing, smell, near/midrange/far vision, depth perception, color/field of vision and driving; frequent sitting, carrying/pushing/pulling, reaching; and occasional climbing, stooping, kneeling, crawling, manual handing and bending. Working conditions include constant exposure to extreme weather, extreme heat/cold, wet/humidity and occasional exposure to noise, vibration and air quality. Potential hazards include constant patient/client contact; frequent exposure to chemicals, needles/syringes and infectious exposure; and occasional exposure to moving mechanical parts and medical equipment.

REQUIREMENTS:

Education: High School diploma or equivalent required.

Experience: Work experience as home care aide or nurse's aide preferred.

Certification/License: Nursing Aide Certification preferred. Will be required to undergo and successfully pass a background check. Must have a valid driver's license and be insurable by the Sault Tribe Insurance Department. Must comply with annual driver license review and insurability standards with the Sault Tribe Insurance Department. Must comply with the Sault Tribe's Drug-Free Workplace Policy which may include random drug tests.

Knowledge, Skills, and Abilities: Knowledge of CPR preferred. Knowledge of community resources preferred. Must maintain confidentiality. Must have reliable transportation and be willing travel extensively throughout community. Must be able to establish and maintain an effective working relationship with supervisory personnel, coworkers and the general public. Must have excellent communication skills and be able to communicate clearly in person, in writing, and by telephone and email. Native American preferred.
