Title: Intake Coordinator

The GPD Intake Coordinator is responsible for coordinating and completing the intake and admission process for veterans entering the Grant and Per Diem (GPD) program. This position ensures that all new admissions are processed efficiently, accurately, and in accordance with program requirements, organizational policies, contractual obligations, and applicable VA/GPD standards.
The Intake Coordinator serves as an important point of contact for veterans, referral partners, case managers, Resident Managers, and program leadership. The position is responsible for preparing intake documentation, verifying eligibility, coordinating bed assignments, maintaining accurate records, and ensuring that veterans receive a structured and welcoming introduction to the program.
Essential Functions and Duties
- Identify housing barriers upon intake including but not limited to tenant history, financial history, social support, criminal history, employment history and other presenting barriers.
- Assist with maintaining accurate program rosters, census reports, and tracking logs.
- Send out morning census of program rosters daily.
- Ensure documentation complies with organizational, contractual, VA/GPD, and privacy requirements.
- Assess risks and appropriateness of applicants and provide recommendations/concerns to Program Manager.
- Establish and maintain both an intake schedule and if needed, a waiting list for prospective clients.
- Respond to queries from prospective clients and referral sources (VA), in timely manner.
- Coordinate scheduled GPD intakes from referral through admission.
- Communicate with referral sources regarding scheduled admissions, missing information, and changes to intake appointments.
- Prepare the appropriate intake packet prior to the veteran's arrival.
- Conduct or assist with the completion of the intake process.
- Explain program expectations, policies, procedures, resident responsibilities, and available services.
- Notify assigned Case Managers of new admissions and provide required intake information.
- Coordinate handoffs from Intake to Case Management and supportive services.
- Ensure veterans receive required orientation information and program materials.
- Coordinate with appropriate staff regarding transportation, arrival times, accommodation, and other intake needs.
- Confirm that the assigned bed/unit is appropriate and ready prior to the veteran's arrival.
- Maintain accurate bed assignments and update the program roster immediately following an intake.
- Other duties as assigned by supervisor
Qualifications:
- Minimum of a bachelor’s degree in human services or social work or related field preferred
- At least 6 months paid experience in Human Services field preferred
- Must be proficient with Microsoft Word and Excel
- Must have good data entry skills
- Knowledge of HMIS a plus.
- Prefer prior experience working with homeless population
- Knowledge of community resources for veterans preferred
- Basic understanding of veterans’ benefits and issues
- Must be able to comfortably, both mentally and physically, perform all aspects of job description
- Must be able to sit comfortably for long periods
- Valid California driver’s license, current vehicle insurance
- Must be able to pass criminal background check and drug screen
- Veteran Status a Plus but not required
Physical Requirements:
- Ability to bend and lift light objects
- Ability to sit for extended periods of time while viewing a computer monitor
- Dexterity of hands and fingers to operate a computer keyboard
- Capable of hearing and speaking to exchange information
- Seeing to read reports and data
- Ability to speak English
- Must be able to work in a noisy, busy environment.
