Job Type: Full-Time (40 hours per week)
Pay:$45,000–$48,000 per year
Schedule: Monday–Friday with occasional evenings and weekends as needed
Work Location: In-person (3300 S. Jefferson Avenue, St. Louis, MO 63118) with occasional local travel throughout the St. Louis area. Mileage reimbursement provided.

Position Summary

Guardian Angel Settlement Association is seeking a compassionate, organized, and community-focused Graceful Guardians Case Manager to provide individualized case management services for older adults throughout the St. Louis community.

The Case Manager will assess participant needs, develop individualized service plans, connect participants with community resources, and provide ongoing support that promotes independence, stability, and successful aging in place.

Using a strengths-based and trauma-informed approach, the Case Manager will help participants overcome barriers related to:

  • Housing stability
  • Food insecurity
  • Healthcare access
  • Transportation
  • Financial well-being
  • Social isolation

This position serves as a trusted point of contact for participants while collaborating with community partners and internal GASA programs to ensure holistic support.

Essential Responsibilities

Case Management & Participant Support

  • Conduct comprehensive intake assessments to identify participant strengths, needs, and goals.
  • Develop individualized service plans with measurable goals and timelines.
  • Provide ongoing case management through regular meetings, phone calls, and follow-up visits.
  • Complete quarterly reassessments and monitor participant progress toward SMART goals.
  • Assist participants with applications for Medicaid, SNAP, utility assistance, and other public benefits.
  • Connect participants to healthcare, housing, transportation, and community resources.
  • Provide crisis intervention, advocacy, and problem-solving support.
  • Assist participants in obtaining identification documents and other required records.
  • Encourage participation in Graceful Guardians workshops, educational programs, outings, and social activities.

Resource Navigation & Community Partnerships

  • Coordinate referrals and provide warm handoffs to community partners.
  • Maintain current knowledge of community resources available to older adults throughout the St. Louis area.
  • Collaborate with healthcare providers, housing agencies, transportation providers, and nonprofit organizations.
  • Monitor referral outcomes and provide follow-up support.
  • Promote participant empowerment and self-advocacy.

Documentation & Program Outcomes

  • Maintain accurate participant records, assessments, case notes, referrals, and service plans.
  • Document participant contacts and progress using the agency's case management system.
  • Assist with tracking program outcomes and grant reporting requirements.
  • Ensure participant files remain compliant with agency policies and funding requirements.
  • Support program evaluation and continuous quality improvement efforts.

Collaboration & Team Support

  • Partner with internal programs including food pantry, trauma response, administration, and early childhood education.
  • Work closely with the Graceful Guardians Program Coordinator to ensure continuity of care.
  • Participate in staff meetings, case consultations, and program planning.
  • Support volunteers and interns assigned to the program.
  • Demonstrate a trauma-informed, participant-centered approach that promotes dignity, respect, and independence.

Qualifications

Education

  • Bachelor's degree in Social Work, Human Services, Gerontology, Public Health, or a related field.
  • Equivalent education and relevant experience may be considered.

Experience

Preferred:

  • Two (2) years of case management, resource navigation, or direct service experience working with older adults or underserved populations.

Knowledge & Skills

  • Strong understanding of challenges impacting older adults, including healthcare access, housing, transportation, food insecurity, and social isolation.
  • Excellent organizational, documentation, and time management skills.
  • Strong written and verbal communication skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Proficiency with Microsoft Office and electronic case management systems.
  • Comfortable learning new technology and software.

Additional Requirements

  • Valid driver's license.
  • Reliable transportation.
  • Proof of automobile insurance.
  • Ability to travel throughout the St. Louis metropolitan area.
  • Knowledge of budgeting, grant requirements, and fiscal accountability is a plus.
