Job Description

Overview:

Responsible for oversight and coordination of all daily activities of volunteers and staff in his/her charge on and off the day hab site.

Responsibilities:

Primary duties and responsibilities include but not limited to:

 

  • Assumes total responsibility for daytime management, care and oversight of select group of individuals who reside in agency residences. Directly supervises Lead HSAs and HSAs.
  • Implements all service activities, ensures supports and adheres to all protective oversights as detailed in the individuals’ records.
  • Creates meaningful and functional service activities according to established agency and program policies, procedures and standards.
  • Develops a staff action plan in coordination with the individual(s) team
  • Provide essential information to all appropriate staff regarding the creation of the CFA/ISP, Protective Oversight, Staff Action Plan and clinical needs as requested. Ensures the accuracy of all reported information.  Protects sensitive information as detailed in agency HIPAA policies and procedures.
  • Ensures health and safety of individuals and adheres to all agency policies and procedures. Ensures emergency treatment if and when needed when in the home or in the community.
  • Ensures maintenance of individual’s records and related documentation.
  • Reports essential information to residence, program supervisors and all other appropriate personnel as necessary.
  • Attends all in-service training and demonstrates proficiency in same.
  • Assists in the orientation and training of PWW staff as needed and upon request of supervisor.
  • Performs additional responsibilities upon request of supervisor.

 

 Ensure that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be  immediately reported to supervisor and/or safety committee.

 

 

Qualifications:
  • High school diploma or equivalent; Bachelor’s degree in Human Services related field preferred
  • Minimum one year experience supporting people with intellectual and other developmental disabilities
  • good oral and written communication skills
  • Qualified NYS driver license and needs to be and remain a Qualified Driver
  • Computer skills preferred
  • AMAP/CPR/First Aid/SCIP-R and defensive driving training preferred.

Physical Aspects may include but are not limited to lifting and transferring of an adult participant.
