General Summary The Health Advocate is responsible for direct oversight and management of all the health needs of the youth we serve in the Residential and Independent Living Programs. This includes medication administration, operations, maintenance, policies, procedures, staff training, youth groups, and overall critical functioning for all medications utilized on grounds at Chase Home. Essential Job Functions - Support, schedule, and attend all medical and dental appointments to meet each youth's needs.
- Attend psychiatry appointments as appropriate.
- Establish and maintain a strong working relationship with Health Direct.
- Maintain adequate amounts of prescribed medications and refill prescriptions or follow up with doctors/medical providers as needed and relay necessary information to all staff (example- when medications/doses change in any way).
- Obtain medical orders/prescriptions as needed for each youth and oversee all medical needs in conjunction with treatment team members and relay necessary information to all staff.
- Communicate to all staff each week a medication count for each youth.
- Responsible for ensuring all youths medical charts in Apricot are current with necessary paperwork including physicals, immunization records, and dental records.
- Responsible for completing monthly reports on health needs for each youth.
- Oversight of all medical logs daily and correcting any mistakes in real time.
- Responsibile for overseeing and filing all Medical Occurences in Apricot.
- Responsible for maintaining adequate supplies of drug tests along with reviewing and communication test results to the Clinical Coordinator and treatment teams.
- Responsible for ensuring staff earn and maintain the appropriate amount of training to satisfy NH DHHS and Child Care Licensing regulations for medication handling and administration.
- Responsible for creating and actively updating all policies and procedures related to medication management and health needs of youth.
- Conduct youth groups pertaining to health education as part of the wellness group curriculum.
- Attend treatment team meetings as necessary particularly when health advocacy is in the best interest of the youth.

Preparation, Knowledge, Skills, and Abilities - 2-3 years related administrative experience desired but not required for the right candidate

Pay Range - $50,000-$56,000 Annually
- Approximately $8,000 in benefits paid by The Chase Home
