Health Coach (Medical)

We are looking for a remote Wellness Coach!

SUMMARY

Remote position responsible for supporting health promotion and wellness programs that encourage members to adopt healthy lifestyles. Provides assessment, health education, diabetes education and coaching, and behavior change counseling using a collaborative approach to improve self-management, medication adherence, lifestyle modification, and clinical outcomes for a diverse member population.

EXPERIENCE

• Two (2) years of experience in a health promotion/education setting, counseling on health topics such as nutrition, exercise, cholesterol, hypertension, stress, alcohol and smoking.
• Two (2) years of experience in delivering chronic condition coaching or diabetes education.
• Knowledge of NCQA, CMS, URAC, HEDIS standards as applicable to designated department assignment.

EDUCATION AND CERTIFICATIONS

• 1 of the following - Certified Health Education Specialist (CHES) certification, Licensed Registered Nurse (RN), or American College of Sports Medicine (ACSM) certification.
• Certified Diabetes Educator Specialist (CDE) or Certified Diabetes Care and Education Specialist (CDCES) preferred
• Bachelor's degree in health education preferred

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Assist in identifying and developing member health education needs through health assessment activities, including online health risk appraisals and telephonic outreach.

2. Provide telephonic outreach, counseling, and coaching on nationally accepted health prevention guidelines, diabetes condition management, and general health and wellness strategies.

3. Serve as a liaison between internal and external stakeholders to champion and develop prevention, wellness, and disease management programs.

4. Monitor, evaluate, and report program effectiveness and participate in continuous improvement activities.
