BRIEF DESCRIPTION:

The purpose of this position is to implement the Health Promotions Program. This is accomplished by working with targeted communities to provide mitigation activities, to include information sharing and learning opportunities, through targeted health promotions. These activities focus on improving the health of the community regarding healthy lifestyles.

This position works with other members of the Health Promotion's staff and is primarily responsible for the domains involving environmental approaches and health system interventions. This is accomplished through the development and implementation of community-based interventions, identifying and engaging community partners and working with interested entities. Additionally, this position will build cross-sector partnerships focused on health equity, identifying appropriate health promotion activities based on assessment feedback to include education and outreach specific to tobacco use, diabetes prevention and education, chronic disease self-management, and pain management, at a minimum.

Other duties include serving as a resource to internal and external stakeholders, participating in various coalitions and committees related to program activities and attending of various meetings, training and planning sessions. Also, coordinating efforts with identified partners to provide resource referrals to community members.

Driver's License Required - Y Type - C

OTHER REQUIREMENTS:

Ability to work evenings, weekends and extended hours.

This position will also be required to provide support to future pandemic response and recovery objectives, as assigned by their supervisor.

# Code Essential Functions

1 L Regular and reliable attendance in compliance with given schedules is a mandatory requirement of this position. Schedules may be subject to change with or without advance notice

2 L Provides health education to include diabetes self-management, diabetes prevention and tobacco cessation classes for the community. Additional tasks are ensuring visuals are available, attending continuing education activities and training sessions.

3 L Increase public and provider knowledge of the symptoms, risk factors and target goals for chronic diseases and the importance of physical activity and healthy eating in preventing, delaying, or managing chronic diseases and its complications.

4 L Develops and presents health education materials for training workshops, conferences, and school or community presentations.

5 L Identifies, designs and implements an intervention, with the community, aimed at addressing the needs of targeted communities.

6 L Raises community awareness to prevent and reduce the prevalence of chronic diseases by promoting health, and support and reinforce healthful behaviors county-wide in schools, worksites, and communities.

MINIMUM JOB REQUIREMENTS:

  • Description of Minimum Job Requirements-

Experience-Related Requirements Over one year and up to and including two years.

Vocational/ Educational Preparation Requires Bachelor's degree or education and training equivalent to four years of college education in business, liberal arts, or any field other than engineering or the hard sciences.
