Provide education, testing, harm reduction information, and referrals to individuals served by the program, as required by program guidelines and directed by the Manager/Supervisor. Conduct online and street-based outreach, and work closely with program staff, other agency staff, and community partners.• Conduct outreach (online and in-person) and health education (1:1 and in group settings) to high-risk communities including LGBTQ+ and unhoused.• Offer and provide HIV, Hepatitis C, and STI education, counseling, and testing services to high-risk folks.• Provide harm reduction education and prevention materials in community settings, encampments, and other field-based locations.• Conduct online outreach on various platforms and street-based outreach in high-need areas• Work with individuals to assess risk, health needs, and readiness for services, and enroll eligible participants into program and clinical services.• Provide referrals and linkages to medical care, substance use treatment, mental health services, HIV/HCV treatment, STI treatment, PrEP, and other support services.• Support and participate in community-based harm reduction and prevention initiatives and outreach events.• Collect, track, document, and enter participant and program data in required systems to support funding requirements, program evaluation, quality improvement, and internal and external program reporting.• Monitor progress toward program objectives and recommend updates or improvements as needed.• Serve as liaison and assist with collaborative activities with community agencies, local government representatives, healthcare providers, and other VCC staff.• Attend meetings, trainings, and community events as needed• Enhance professional growth and development through participation in trainings, current literature review, in-service meetings and workshops• Support the vision, mission and goals of the organization and demonstrate commitment to its values.
Minimum
• High school graduate or equivalent• Minimum of one year experience in community education• Bilingual English/Spanish• Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record• Ability and willingness to work flexible hours, including evenings and weekends
