Job Summary
The Health Mentor is responsible for working with clients enrolled in the InSHAPE program to develop and implement their personal wellness plan. They will provide support, education, and motivation related to physical fitness, nutrition, and wellness to clients in the community. The Health Mentor will model appropriate wellness and provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.
The Health Mentor must have a personal trainer certification or must obtain a personal trainer certification within 6 months of being hired.
Responsibilities
Collaborate with clients and program employees to design and implement individual wellness plans.
Provide fitness training, monitoring, and continued reassessment of exercise, nutrition, and health commitments.
Administer fitness assessments to establish client baseline and record data.
Explain and demonstrate proper training techniques and exercises; correct form to prevent injury; and provide modifications, as necessary.
Lead group activity instruction/fitness sessions for clients and employees as requested/needed.
Conduct nutrition counseling; i.e. create food journals/grocery lists, provide recipes, grocery shop with clients, cook with clients, etc.
Provide ongoing health and fitness education that motivates continued movement of wellness lifestyle changes and goals.
Encourage and support member participation in the program and empower clients to lead a healthy life.
Provide consultation to employees/teams regarding client’s health goals, progress, and planning.
Perform other related duties and/or projects as assigned.
Complete daily progress notes based on services provided to clients in a timely manner reflected in Trilogy’s documentation policy and agency standards.
Engage regularly in team meetings to consult and collaborate on caseloads.
Qualifications
Must have one of the following requirements:
Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
Licensed Practical Nurse under the Nurse Practice Act [225 ILCS 65]
Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience of experience in a mental health setting
CRSS certification through IAODAPCA
Family Partnership Professional certificate from and in good standing with IAODAPCA
Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with one-year experience in a mental health setting;
High school diploma or GED and five years of supervised clinical experience in a mental health setting
Valid Illinois driver’s licenseâ¯withâ¯daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liability insuranceâ¯â¯
Personal trainer certification or must obtain personal trainer certification within 6 months of hire date.
CPR certification or must obtain CPR certification within 6 months of hire date.

Experience and or training in motivational interviewing and motivational strategy building
