Program Overview
The Young Men Who Have Sex with Men (YMSM) Program provides comprehensive, community-based HIV prevention, sexual health education, and navigation services for young men who have sex with men. The program utilizes evidence based, harm reduction, and culturally responsive strategies to reduce HIV and HCV transmission, increase access to biomedical prevention, and address barriers to care.
Purpose of the Position
The Health Navigator / Outreach Worker is responsible for engaging YMSM through targeted outreach, delivering rapid testing services, facilitating group-level interventions, supporting drop-in programming, and providing navigation and linkage to HIV prevention and care services. This role ensures participants have access to prevention resources, supportive services, and a welcoming, affirming space.
Key Responsibilities
Outreach & Community Engagement
• Conduct community outreach a minimum of three (3) times per week, specifically targeting YMSM, in physical and virtual spaces.
• Identify and prioritize outreach efforts in areas with high HIV infection rates using local NYS ETE dashboard data.
• Provide culturally affirming education on HIV, HCV, STI prevention, sexual health, and risk-reduction strategies.
• Work at least one (1) Saturday per month and one (1) evening per month to support outreach or agency-wide programming.
Testing & Screening Services
• Provide rapid HIV and Hepatitis C (HCV) testing in community-based, wellness center, and mobile health unit settings.
• Conduct pre- and post-test counseling per NYSDOH and agency protocols.
• Ensure informed consent, confidentiality, and accurate documentation.
Navigation, Referrals & Linkage to Care
• Provide health navigation to support access to PrEP, PEP, STI testing, HIV/HCV medical care, and supportive services.
• Address barriers such as insurance, transportation, and system navigation.
• Facilitate warm handoffs and follow-up.
Group-Level Interventions & Education
• Co-facilitate evidence-based intervention such as, Popular Opinion Leader.
• Support recruitment and engagement for group activities and workshops.
Drop-In Activities & Wellness Center Support
• Support and facilitate drop-in activities designed to promote engagement, retention, and community building among YMSM.
• Provide informal education, resource navigation, and referrals during drop-in hours.
• Ensure the wellness center is stocked and organized with essential materials, including but not limited to:
• MetroCards and transportation assistance
• Snacks and beverages
• Hygiene kits and safer sex supplies
• Educational and outreach materials
• Monitor inventory levels and communicate restocking needs to supervisors.
• Maintain a welcoming, affirming, and safe environment during all drop-in programming.
Data Entry, Documentation & Quality Assurance
• Enter all outreach, testing, navigation, and drop-in service data by the end of each business week.
• Perform self-audits of testing forms and service documentation.
• Maintain confidentiality and adhere to HIPAA, NYSDOH, and agency policies.
Training & Professional Development
• Attend a minimum of twelve (12) trainings per contract year.
• Participate in supervision, staff meetings, and professional development.
