Health Plan Nurse Coordinator (RN)- 100% Remote

Overview

We"re seeking an experienced Registered Nurse (RN) to support care management, utilization review, and population health programs. You"ll work closely with members, providers, and interdisciplinary teams to improve health outcomes and ensure members receive appropriate, coordinated care.

Responsibilities

• Conduct utilization review and care management activities.
• Coordinate care transitions and support population health initiatives.
• Collaborate with providers, members, and care teams.
• Maintain accurate clinical documentation and member records.
• Ensure compliance with HIPAA, regulatory, and health plan requirements.
• Identify quality-of-care concerns and participate in quality improvement efforts.
• Manage multiple priorities while meeting established deadlines.

Required Qualifications

• Active, unrestricted RN license.
• Experience with Utilization Management (UM) or Utilization Review.
• Strong clinical assessment and care coordination skills.
• Excellent communication and organizational abilities.
• Knowledge of HIPAA and healthcare compliance requirements.
• Ability to work independently and collaboratively.

Preferred Qualifications

• Experience in Case Management, Disease Management, or Population Health.
• Health plan, hospital, or provider-based care management experience.
• HEDIS abstraction experience.
• Bilingual Spanish preferred.

Job Type & Location

This is a Contract position based out of Santa Barbara, CA.

Pay and Benefits

The pay range for this position is $47.00 - $47.00/hr.
