Sentara Health is hiring for an Health Record and Information Technician I!

Status: Full-Time, permanent position (40 hours)

Standard working hours: 9am to 530pm EST, M-F (fixed shift)

Location: Hybrid

Job Description Summary

The Health Record and Information Technician I is responsible for the interpretation and preparation of all documents for precision scanning into the electronic medical record with strict attention to maintaining patient confidentiality guidelines for safeguarding patient medical information. This role is required to work within many different computer environments to
track, scan, identify, validate, and access medical records. The Health Record and Information Technician I is responsible for maintaining data and statistics to support functions within the department and for the needs of patients, providers, and other applicable entities.

Description 

Review and analyze documents for the electronic medical record with strict attention to maintain patient confidentiality guidelines for safeguarding patient medical information. Ensure documents are readily available in the electronic medical record for patient care and medical decisions. Required to work within many different computer environments to track, scan, identify, validate, and access medical records. Responsible for maintaining data and statistics to support activities. Assist with release of information requests.  

  

Key Responsibilities 

Health Information Management & Record Processing 

  • Retrieve, assemble, and analyze medical records for completeness, accuracy, and compliance with regulatory standards. 

  • Maintain electronic health record (EHR) systems by scanning, indexing, and validating documents in appropriate patient charts. 

  • Review provider documentation for required signatures, dates, and authentication. 

  • Process record requests, ensuring compliance with HIPAA and organizational policies regarding release of information. 

Data Quality & Integrity 

  • Verify accuracy of patient demographic and encounter data in EHR and health information systems. 

  • Conduct quality checks on scanned and uploaded documents to ensure legibility, correct patient matching, and completeness. 

  • Assist with chart audits and data validation for internal quality, coding, and compliance reviews. 

  • Support updates to patient information to maintain consistent data integrity across clinical, billing, and reporting systems. 

Confidentiality, Security & Compliance 

  • Maintain strict confidentiality of patient health information in accordance with HIPAA and internal privacy policies. 

  • Report any potential breaches, discrepancies, or security issues to the appropriate compliance or privacy officer. 

Customer Service & Communication 

  • Provide courteous and timely responses to internal and external requests for patient information. 

  • Collaborate with physicians, nurses, coders, and other healthcare staff to resolve documentation or record-related issues.

  • Communicate effectively with release-of-information vendors, legal representatives, and external agencies as needed. 

Other Requirements 

  • Good hand-eye coordination and the ability to meet process document standards as set.  

    Ability to focus on detailed processes and validate accuracy of others

Education:

  • HS - High School Grad or Equivalent REQUIRED

Certification/Licensure:

  • None required

Experience:

  • 1 year experience in Health Information Services – REQUIRED

  • Graduate of an approved Health Information Technician or Health Information Systems training program is accepted in lieu of experience (HRIT Certification or CCA or CCS (Coding) or AHIMA )

  • EPIC experience is preferred

  • Onbase experience is a plus
