Job Description and Duties

Metropolitan State Hospital is located in Norwalk, Los Angeles County.

The Department of State Hospitals - Metropolitan, Health Information Management Department (HIMD), is accepting applications for the Health Record Technician I (HRT I), Permanent/Full Time position. Under the direction of the Supervisor I or designee, the HRT I performs a variety of general office and clerical work in support of the Hospital's Vision, Mission, Goals, and Values. The HRT I is expected to consistently exercise a high degree of self-initiative and independence when performing assigned tasks. Responsibilities include but are not limited to:

CONCURRENT:

Abstract, process, file, collate, and audit data from health records. Enter abstracted health record data into the Admissions/Discharge/Transfers (ADT) computer system and PLATO. Chart Maintenance: Label, repair and arrange. Code health records in accordance with DSM, ICD, and CPT coding guidelines: such as discharge summaries, psychiatric evaluations, ancillary services, treatment planning conferences, physician and psychologist billing services. Maintain log of completed work and perform other special audits and reviews related to health records functions. Deliver and retrieve health records between patient care areas and HIMD.

DISCHARGE:

Abstract, process, file, collate, and audit data from health records. Enter abstracted health record data into the ADT computer system. Code health records in accordance with DSM, ICD, and CPT coding guidelines: such as discharge summaries, psychiatric evaluations, ancillary services, treatment planning conferences, physician and psychologist billing services. Locate, retrieve and file health records and documentation in all record storage areas. Maintain and purge files/documents in all archive areas. Process health records for admission, transfer or discharge. Maintain assigned filing areas in correct filing order for easy retrieval and access of health records. Assemble health records as a unit record; all prior admissions brought forward and filed under the new hospital admission number. Deliver and retrieve health records within the hospital and copy health records as assigned. Maintain a log of completed work and perform other audits and reviews related to health record functions.

Training and Development (T&D): Individuals who are eligible for a T&D assignment may also be considered for this position. T&D assignments may be considered for current State employees. If you are a current State employee interested in a T&D assignment, you are not required to meet the Minimum Qualifications; however, you must indicate T&D as your eligibility when applying. Please see the "Training and Development (T&D) Information" section below for more details.

Minimum Qualifications:

Either I: One year of California state service clerical experience which includes training and supervised work experience in the procurement and processing of health data.

Or II: Two years of clerical experience in the medical or health records section of a health facility. (Successful completion of an academic curriculum in medical records science in an accredited school may be substituted for the required general experience on a year-for-year basis.)

Opened in 1916, DSH-Metropolitan State Hospital is in the vibrant community of Norwalk, 15 miles east of Los Angeles. To the south are entertainment venues like Disneyland, Knott's Berry Farm, the marinas and beaches of Huntington, Newport and Laguna. To the west are Palos Verdes and Marina Del Rey.

DSH-Metropolitan State Hospital has an open campus within a security perimeter. The hospital operates approximately 826 beds. The commitment categories of patients treated at DSH-Metropolitan: Lanterman-Petris-Short (LPS) Act, Incompetent to Stand Trial (PC 1370), Offenders with Mental Health Disorders (Penal Code Section 2964/2972/2974), Not Guilty by Reason of Insanity (PC 1026).

Training and Development (T&D) Information

T&D assignments are temporary assignments of up to 24 months for purposes of training and providing career advancement opportunities to prepare permanent civil service employees for future promotions into new occupational fields. DSH encourages the use of T&D assignments to provide employees with the opportunity to obtain any or a combination of the following:

  • Experience necessary to satisfy the experience component of the minimum qualifications of the Training and Development classification or to gain preferred or desirable competencies.
  • Broader or more specialized competencies that will improve the employee's performance in their current position and classification.
  • Experience that would prepare the employee for future promotion.
  • Experience to facilitate the employee's entry into a new occupational or career-related field.

Applicants interested in applying as a T&D assignment must have permanent status in their "current class" to be eligible. An employee currently in a Limited Term position is not eligible for a T&D assignment. The employee may, but is not required to, satisfy the minimum qualifications of the T&D classification; except, the employee shall possess any licensure, certificate, or similar credential that is a component of the minimum qualifications.

Training and Development (T&D) assignments may be considered for this position for current California civil service employees. T&D assignments cannot exceed 24 months. However, successful T&D candidates may transition to a permanent appointment, if eligible at the end of the T&D assignment.
