The SSVF Healthcare Navigator connects Participants to the VA for Health Care or Community Health Care Services. Bachelors Degree preferred or High School Diploma and 4 years experience. Provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties.
