Position Summary:
The HealthySteps Specialist/CHW provides support to caregivers of patients 0-3 years old seen in the El Nuevo San Juan Health Center Pediatric Dept identified as needing support based on patients screening positive on evidence based screening tools or those referred by their provider for additional support.
Provides direct services to the parents/caregivers of at-risk children providing support and referrals for needed services and consultation to the providers. Attends required meetings and trainings ensuring fidelity to the evidence based HealthySteps Program. Collects and tracks program data and participates in meetings/trainings as part of the interdisciplinary pediatric and quality improvement team at UHP.
This is an onsite role with a schedule of Monday through Friday, 8am-5pm based in the Bronx.
Minimum Qualifications:
- Bachelor''s Degree in Social Worker or Early Childhood Education is required.
- 1-2 years of experience within healthcare, mental health, or social services.
- Ability to diagnose patients.
- Bilingual Spanish/English is required.
- Excellent interpersonal and patient engagement skills.
Key Duties:
- Provides support to caregivers of patients 0-3 years old seen in the El Nuevo San Juan Health Center Pediatric Dept identified as needing support based on patients screening positive on evidence-based screening tools or those referred by their provider for additional support.
- Provides direct services to the parents/caregivers of at-risk children providing support and referrals for social, behavioral health and support services for needed services working closely with primary care pediatric providers and the pediatric team members.
- Performs screenings as assigned including but not limited maternal depression screening and ensures appropriate referral to behavioral health based on screening results.
- Supports patient engagement in early intervention for patients identified as needing those services working closely with the pediatric health educator.
- Collects and tracks program data and participates in meetings/trainings as part of the interdisciplinary pediatric and quality improvement team at UHP.
- Maintains a caseload of parents/caregivers providing ongoing social and support services to ensure engagement in needed services.
- Attends required meetings and trainings ensuring fidelity to the evidence based HealthySteps Program.
- Collects and tracks program data and participates in meetings/trainings as part of the interdisciplinary pediatric and quality improvement team at UHP.
- Documents all contacts in patient chart and adheres to all regulations including HIPAA confidentiality regulations.
Starting Compensation: Minimum $47,000 per year; Maximum $50,000 per year
