<\/span>• Experience: Hematology/Oncology Experience Preferred

DUTIES & RESPONSIBILITIES
<\/p>
• Manage patients with solid tumors and hematologic malignancies
<\/span>• Provide outpatient oncology and hematology care
<\/span>• Collaborate with hematologist/oncologists and multidisciplinary care teams
<\/span>• Coordinate patient treatment plans and follow -up care
<\/span>• Document all encounters accurately within Epic EMR
<\/span>• Participate in inpatient collaboration as clinically indicated
<\/span>• Support continuity of care through coordination with infusion, research, navigation, and support services
<\/span>• Ensure compliance with facility policies and quality standards
