Hepatitis C Linkage to Care Specialist

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Full-time Hepatitis C Linkage to Care Specialist to join our team located in Poughkeepsie, NY.

The Hepatitis C Linkage to Care Specialist will work with the Director of Nutrition and Operations to provide education and outreach for patients that are HCV positive

• $1,500 Sign on Bonus*

Responsibilities:

• Facilitates enrollment in HCV Benefits and Entitlement programs
• Maintains data and case records as required and prepares necessary reports
• Education among community agencies and internal staff members about the importance of Hepatitis C testing and treatment
• Educate providers and other staff about HCV testing, benefits of diagnosis and importance of linking to care
• Works cooperatively and collaboratively with entire HCV case management, Medical and Behavioral Health Provider teams
• Provide outreach to testing centers, education those newly diagnosed with Hepatitis C and referral to SRH, including tracking of all services
• Focuses on reducing barriers to ensure timely access to care and treatment.
• Develops linkages and internally refers patients for additional medical services, including treatment
• Work with Community Based Organizations (CBOs) to set up HCV screening services, provide community education and outreach and act as a liaison with CBOs to link HCV positive patients to care
• Participates in case conferences as well as other agency or community-based meetings or trainings

Job Type: Full Time

Education: Associate's Degree (Required) Bachelor's Degree (preferred)

Pay: $22.00 - 25.47/hour
