    Healthcare for the Homeless - Houston HHH Family Clinic Physician and Medical Director

     

    Position summary:

    Healthcare for the Homeless- Houston (HHH) is a Federally Qualified Health Center (FQHC) committed to improving the health and well-being of individuals and families experiencing homelessness.

    As part of HHH's commitment to patients experiencing housing instability, the HHH Reed Family Clinic cares for families experiencing homelessness or housing transitions, from infancy to aging.

    The HHH Family Clinic Physician and Medical Director as a part-time role provides clinical leadership and administrative oversight for the family clinic and the health needs of that specific community, ensuring the delivery of high-quality, patient-centered healthcare services through a family-medicine care model.

    This role maintains a strong clinical role, with a primary focus on direct patient care. Additionally, the HHH Family Clinic Medical Director is responsible for overseeing quality initiatives and best practices implementation specifically focused at HHH Reed Family Clinic, working closely with HHH administrative and clinical leadership for this purpose.

    The role reports to the HHH Chief Medical Officer and Associate Medical Director, and models the delivery of high-quality, patient-centered healthcare services through all family-based clinics operated underHHH.

    Key Responsibilities

    Clinical Leadership & Program Oversight

    • Provide leadership, strategic direction, and operational oversight for the HHH Family Clinic program, ensuring the delivery of comprehensive, high-quality healthcare services.
    • Collaborate with HHH management to maintain clinical standards consistent with FQHC requirements, HRSA regulations, and evidence-based medical care.
    • Deliver direct patient care, as primary scope of position
    • Support development of clinical protocols, quality improvement initiatives, and best practices, as applicable for HHH Family Clinic model
    • Monitor clinical outcomes, patient access, and clinic productivity to ensure clinic effectiveness and sustainability.
    • Work with HHH clinical management team to ensure compliance with infection control standards, OSHA regulations, HIPAA requirements, and other applicable laws and guidelines, as well as academic institution requirements applicable to partnering institutions
    • Lead or support any HHH Reed Family Clinic -related operational meetings
    • Meet with HHH CMO for ongoing oversight
    • As indicated, supervise medical students and physician resident learners
    • Maintain a structured and supportive learning environment that promotes professional development and culturally responsive care, holding to HHH expectations as a clinical preceptor.
    • Oversee any pertinent clinical rotations for medical students in partnership with local academic institutions.
    • Support accreditation efforts, audits, and reporting requirements associated with FQHC operations as applicable for HHH Reed Family Clinic
    • Participate in the Vaccines for Children/Adult Safety Net vaccine program, which may require active engagement and involvement by provider in addition to staff
    • Follow and enforce the HHH established expectations regarding clinical documentation, billing, and compliance.
    • Promote integration of HHH model of care: primary care, behavioral health, and social services.
    • Support engagement efforts targeting individuals experiencing homelessness.
    • Participate with multidisciplinary team in Reed community outreach efforts and engagement, ensuring HHH representation at health fairs and other events
    • Promote connection with local community partners, including regular meetings and representation at community events
    • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
    • Current DEA registration (or ability to obtain).
    • Minimum of three (3) years of clinical medical experience, preferably in community health, public health, or safety-net settings.
    • Prior experience with clinical care for adults, pediatrics and gynecology
    • Experience working with underserved or vulnerable populations, including individuals experiencing homelessness, preferred.
    • Commitment to health equity and compassionate care for marginalized populations.
    • Ability to collaborate effectively within an interdisciplinary healthcare environment.

    Operational Leadership & Education Oversight

    •          Attend HHH Clinical Leadership Committee as scheduled

    •          Collaborate with HHH clinical management for Reed operations

    Quality Improvement & Compliance

    Community & Integrated Care

    QualificationsEducation & Licensure

    •          Active and unrestricted license to practice medicine in the State of Texas.

    •          Board Eligible/Board Certified

    •          Current CPR/BLS certification.

    Experience

    •          Previous leadership, supervisory, or program management experience preferred.

    •          Strong leadership and team management abilities

    •          Confidence in clinically practicing through a trauma-informed lens

    •          Excellent communication, mentoring, organizational, and problem-solving skills.
