About the Role:
San Fernando Medical Center is looking for a talented and compassionate Licensed Physician Assistant or Family Nurse Practitioner to join our dedicated healthcare team in San Fernando, CA. Responsibilities:
  • Conduct comprehensive patient evaluations, including history-taking and physical examinations
  • Diagnose and treat acute and chronic medical conditions across diverse patient populations
  • Develop and implement individualized treatment plans and care management strategies
  • Order and interpret diagnostic tests, labs, and imaging studies
  • Prescribe medications and manage ongoing pharmacological therapies
  • Collaborate with physicians, specialists, and care teams to ensure continuity of care
  • Educate patients and families on health maintenance, disease prevention, and treatment options
  • Maintain accurate and thorough clinical documentation in the EMR system
Requirements:
  • Active California license as a Physician Assistant (PA-C) or Family Nurse Practitioner (FNP)
  • Current DEA certification and prescriptive authority
  • Experience in primary care, family medicine, or a related clinical setting preferred
  • Strong clinical assessment, diagnostic, and critical thinking skills
  • Proficiency with electronic medical records (EMR) systems
  • Excellent communication and interpersonal skills with patients and care teams
  • Bilingual (English/Spanish) a plus given the local patient population
