Sage Group HealthCare Staffing is seeking Home Health OT for a 13-week travel / contract position in Milwaukee, WI. Accepting both local and traveler candidates.

Pay Rate: Range $2200 to $2240 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments

Shifts available: Days
Shift timings: 8 Hours; Days, 8a â 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No weekend or call
Position Requirements:
- Current Occupational Therapist experience: 1-2 years - Required
- Recent/Current Home Health OT experience: 12 months within the past 3 years - Required
- Home Care Home Base (HCHB) charting experience - Required
- Must have valid Driverâs License and Personal Auto Insurance
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
- Bachelor of Science degree in Occupational Therapy or Similar â Required
- Current State License as Occupational Therapist for the state applying or compact if accepted in that state - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide Occupational Therapy services to patients/clients at their homes.
- Consult with the physician and an interdisciplinary team in the development of a plan of treatment
- Utilize skills in patient/client evaluation and treatment, professional consultations to achieve maximum patient/client independence in accordance with the individual's functional level and potential
- Plans and prepares treatment plan based on evaluation of patient data
- Provide in-home OT treatment programs to assist patients in restoring function, relieving pain, and preventing disability
- Allow patients and their families to maintain dignity and independence while receiving safe, effective rehabilitation in the security of their home
- Drive to patient homes in the assigned territories on daily basis.
