    Description

    Sage Group HealthCare Staffing is seeking a Physical Therapist I for a permanent/full-time position in Michigan. New graduates are encouraged to apply.

    Position Details:

    • Position Name: Physical Therapist I
    • Location: Tawas City, Michigan
    • Specialty: Physical Therapy (Outpatient & Acute Care)
    • Start Date: ASAP
    • Employment Type: Full-time, Permanent
    • Pay Rate:Starting at $37.00/hour (Based on experience)
    • Sign-On Bonus:$20,000 (2-year commitment required)
    • Primary Practice Area: Outpatient with some Acute Care coverage
    • Shift: Days (Based on department needs)

    Position Requirements:

    • New graduates are welcome to apply
    • Strong clinical assessment, treatment planning, and rehabilitation skills
    • Ability to independently manage patient care plans and treatment programs
    • Excellent communication, organizational, and problem-solving skills
    • Ability to supervise Physical Therapist Assistants (PTAs), aides, and students
    • Ability to work collaboratively with interdisciplinary healthcare teams
    • Commitment to providing high-quality, patient-centered care

    Education, License & Certification Requirements:

    • Bachelor’s Degree in Physical Therapy – Required
    • Active Physical Therapist (PT) License – Required
    • BLS Certification – Highly Preferred (within one month of hire)
    • Completion of required orientation and competency programs

    Job Description:

    • Evaluate patients through formal and informal assessments to determine functional abilities, including skeletal, neurological, respiratory, cardiovascular, and mobility status.
    • Develop individualized treatment plans based on assessment findings, physician orders, and patient goals.
    • Provide physical therapy interventions to restore, improve, and maintain optimal functional abilities.
    • Educate patients, families, and caregivers regarding diagnoses, treatment plans, home exercise programs, safety strategies, and rehabilitation goals.
    • Supervise Physical Therapist Assistants, Physical Therapy Aides, Home Health Aides, and students according to professional and regulatory standards.
    • Recommend adaptive equipment and assist patients with obtaining appropriate supportive devices.
    • Complete accurate and timely documentation required for patient care, billing, and regulatory compliance.
    • Monitor patient progress and modify treatment plans based on changes in patient condition.
    • Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate effective patient care.
    • Participate in interdisciplinary team meetings, quality improvement initiatives, and program development.
    • Serve as a case manager for assigned patients when required.
    • Promote physical therapy services through patient education and community outreach.
    • Maintain patient confidentiality and follow all safety, infection control, and compliance standards.
    • Perform additional duties as assigned.
