Position Title:HIV Quality Management Coordinator

PayGrade:C-2

POSITION SUMMARY:

With minimal supervision, carries out assigned responsibilities of the HIV Program. Duties include utilizing applicable client level databases to run data queries, analyze and report findings through various platforms, compare data sets to grant deliverables to ensure compliance, utilize findings to inform quality improvement initiatives, facilitate quality improvement meetings, and oversee quality management projects.

SUPERVISION EXERCISED:

Provides no direct supervision of Health Department staff.

SUPERVISION RECEIVED:

The position is mostly self-directed with general supervision by the HIV Program Manager.

Examples of Duties

ESSENTIALFUNCTIONS:

• Collect and review quantitative and qualitative programmatic data, organize data into a presentable format, analyze and report findings to HIV Care & Prevention programs and community stakeholders, with recommendations for improvement.
• Coordinate and oversee planning, implementation, and data review of quality improvement projects, providing directive, guidance, and encouragement.Utilize information learned during the Plan, Do, Study, Act process to inform regional policy development.
• Plan and facilitate regional meetings including, but not limited to, Quality Management Committee and Consumer Advisory Committee.
• Conduct quarterly regional and agency specific client level chart audits and prepare finding reports. Monitor Corrective Action Plans for identified deficiencies as applicable.
• Conduct annual programmatic site visits with subcontracted providers and vendors to ensure contract compliance and prepare site visit reports. Monitor Corrective Action Plans for identified deficiencies as applicable.

Typical Qualifications

JOB REQUIREMENTS AND QUALIFICATIONS:

Requires a bachelor's degree from an accredited university in epidemiology, behavioral science, social science, or a health-related field with one (1) year of experience.

-OR-

An associate degree from an accredited university and two (2) years of experience.

Experience in infectious disease services and working knowledge of Microsoft Office, Provide Enterprise, Epic, REDCap, I-CARE, and/or other Illinois Department of Public Health (IDPH) recommended platforms is preferred.

ESSENTIAL PHYSICAL/COGNITIVE ABILITIES

• The ability to sit, flex, stand, reach, and bend.
• The physical strength to lift, carry, and transport materials/equipment are equivalent to thirty (30) pounds.
• Visual acuity, auditory ability, and clear written and verbal communication ability.
• Manual dexterity and the ability for data input into various computer programs.
• The ability to maintain professional behavior at all times.

SPECIAL REQUIREMENTS

• Must reside within St. Clair County, IL or be willing to relocate to St. Clair County within six (6 ) months from hire date.

• The ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned.

• May require provision of services at off-site locations.

• A valid Drivers License.

• A dependable automobile for use in position.

• Autoinsuranceineffect with at least the minimum limits of liability as outlined and required by the state of Illinois.

• Adherence to all local, state and federal regulations pertaining to OSHA guidelines.

• Health Department employees:

• Are required to have a TB test upon hire and at separation of employment.

• Must be current on CDC Healthcare Personnel Vaccine Recommendations, which include Hepatitis B, MMR (Measles, Mumps Rubella), Td/Tdap (Tetanus/Diphtheria/Pertussis), Varicella (chickenpox) and Meningococcal if over 65. Influenza and COVID vaccines are recommended and encouraged, but not required at this time and maybe subject to change.

• All employees must adhere to the St. Clair County Personnel Code.

• Position contingent upon continued grant funding.
