Job Title/Position: Home Care Aide
Reports To: Clinical Manager
JOB DESCRIPTION SUMMARY
A Home Care Aide performs tasks thatenable the client to remain in the home by supplementing the client'sabilities for home management.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assists or performs personal care tasks including ADLs and IADLs to enable client to re-
main safe in the home environment, maintain capacity to function, delay disease and pre-
vent regression.
2. Provideshomemaking services for the ill, injured, impaired, disabled or aged.
3. Observes the client for changes in general physical and mental conditions. Reports any
changes noted to the clinical manager or supervisor.
4. Keeps thehome clean and orderly.
5. Plans meals and purchases food and household supplies, as appropriate.
6. Prepares and serves nutritious meals according to the client’sand caregiver'sneeds and the
client's services delivery plan.
7. Washes dishes and cleansthe kitchen after meals.
8. Assists the clientand caregiver in the practice of safety, hygiene, and good nutrition
where possible.
9. Does client’s laundry.
10. Adheres to the documentationand care proceduresand standards ofpersonal and
professional conduct.
POSITION QUALIFICATIONS
1. Is at least eighteen years of age.
2. Has at leastone year of experience as a caregiver, homemaker or home care aide.
3. Has had work or life experience as a caregiver, homemaker or home care aide.
4. Successfully completes the home health aide competency evaluation.
5. Has the ability to readand follow written instructionsand document services provided.
6. Has empathy for the needs of the ill, injured, frail and impaired.
7. Must be a licensed driver with anautomobile that is insured in accordance with state and
organization requirements and is in good working order OR must maintain a safe and
efficient methodof transportation aimed at arriving to work as scheduled.
JOB LIMITATIONS
The home care aide will not function in any manner viewed as the practice of nursing according to the Utah Nurse Practice Act. Specifically, the home care aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and
skill of a licensed nurse.
