Job Summary
We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus. 

Responsibilities:

• Assist clients with daily living activities, including bathing, grooming, and meal preparation.
• Provide companionship and emotional support to enhance clients' quality of life.
• Administer medication and monitor health status as directed by healthcare professionals.
• Help clients with mobility and transportation to appointments or social activities.
• Maintain a clean and safe environment for clients.
• Document and report changes in clients' conditions to the care team.
• Collaborate with family members and healthcare providers to ensure comprehensive care.
• Participate in training and development programs to enhance caregiving skills.

Requirements:

• High school diploma or equivalent; additional caregiving certifications preferred.
• Previous experience in direct care or a related field is highly desirable.
• Valid driver's license and reliable transportation.
• Strong communication and interpersonal skills.
• Ability to lift and assist clients in a safe manner.
• Compassionate and patient demeanor with a passion for helping others.
• Willingness to work flexible hours, including weekends and holidays.
• Background check and professional reference checks required.
