Position Summary


Optimal Home Care Services is seeking a compassionate, reliable, and motivated Caregiver/Direct Support Professional (DSP) to provide individualized support to a young adult receiving disability waiver services in Brooklyn Park. The ideal candidate will enjoy supporting individuals in the community, promoting independence, and providing encouragement with daily activities and personal goals.


Responsibilities

• Provide companionship and social support.
• Assist with community outings, shopping, and recreational activities.
• Encourage participation in walks and healthy lifestyle activities.
• Support personal care routines, including grooming, hygiene, and dressing as needed.
• Assist with meal planning, meal preparation, and healthy eating habits.
• Provide medication reminders and wellness monitoring.
• Promote independence while ensuring health and safety.
• Support employment-related goals by assisting with job searches, applications, and community resources.
• Document services accurately and communicate concerns to supervisors.
• Provide transportation to appointments and community activities when required.

Preferred Qualifications

• Experience working with individuals with mental health diagnoses, developmental disabilities, or waiver services.
• Strong communication and interpersonal skills.
• Ability to encourage engagement and independence.
• Valid driver's license, reliable vehicle, and proof of insurance.
• Ability to pass DHS background studies.
• CPR/First Aid certification preferred.
