Position Overview
- Can you wear ten hats? Our Care Coordinator should be able to ensure every client and caregiver feels supported, heard, and cared for. This role keeps our agency running smoothly by managing schedules, communicating with families, and helping caregivers succeed in the field.
Reports to:
Owner/RN
Performance Criteria:
- Build Schedules that match caregivers with clients based on skills, personality, and availability.
- Optimize Caregiver schedules, reduce gaps, and maintain high visit completion rates
- Support Caregivers with real-time help, encouragement, and problem-solving.
- Communicate with Families to ensure they feel confident and informed.
- Coordinate New Clients by helping them transition smoothly into services.
- Monitor Care Plans and share updates with RN/Owner.
- Maintain Office Flow so daily operations stay organized and predictable.
- Execute Intake Process, documentation, and service activation.

- Strong organizational and time-management abilities
- Experience with scheduling or operations (a must)
- Excellent communication and problem-solving skills
- Ability to work in a fast-paced environment
- 2+years experience in home care staffing company
- Tech-savvy with scheduling platforms (like WellSky) and mobile apps
