As a Care Coordinator, you'll play a vital role in ensuring our clients receive compassionate, reliable care while supporting caregivers and collaborating with our internal team. What You'll Do


  • Coordinate the start of care for new clients and ensure a smooth onboarding process.
  • Maintain accurate client and caregiver records in our software systems.
  • Communicate with clients, families, caregivers, and healthcare professionals regarding care needs and updates.
  • Monitor client satisfaction through routine follow-up calls and quality assurance check-ins.
  • Assist with caregiver-client matching based on client needs and caregiver qualifications.
  • Track required documentation and ensure compliance with company policies and state regulations.
  • Support caregivers with questions, concerns, and ongoing communication.
  • Collaborate with office staff to ensure continuity of care and excellent customer service.
  • Assist with care plan updates and documentation as needed.
What We're Looking For


  • Previous experience in home care, healthcare coordination, case management, or a related field preferred.
  • Excellent communication and interpersonal skills.
  • Strong organizational skills with exceptional attention to detail.
  • Ability to prioritize multiple tasks in a fast-paced office environment.
  • Proficiency with Microsoft Office and the ability to learn new software systems.
  • Professional, compassionate, and customer-service oriented.
  • High school diploma or equivalent required; additional healthcare or administrative experience preferred.
