Schedule: Exempt (salary), min 40 hours per week | Monday - Friday, 8:00 am – 4:30 pm
Department: LifeCircles PACE | Holland, MI
As the LPN Home Care Coordinator, you’ll be a vital part of the interdisciplinary team, helping LifeCircles participants receive the in-home care and support they need to thrive. You will coordinate a variety of home care services—including personal care, homemaking, respite, and community living support as well as durable medical equipment and supplies. You’ll ensure services are delivered efficiently, in line with established procedures, and always with participant well-being in mind.
Key Responsibilities
Participant Care and Support
- Assess and monitor participants’ acute and chronic needs across clinic, home, and facility settings.
- Provide treatments within scope; collect vitals and lab specimens, reporting changes to the RN/provider.
- Assist with activities of daily living (ADLs), including bathing, dressing, and hygiene as needed.
Care Coordination and Collaboration
- Support care plan implementation and communicate changes in condition to the interdisciplinary team.
- Coordinate with team members, caregivers, and external partners to ensure continuity of care.
- Provide participant and caregiver education.
Documentation and Records
- Accurately document care, observations, and participant status in compliance with policies and regulations.
Clinic Operations and Safety
- Maintain a clean, safe clinical environment and follow infection control protocols.
- Assist with equipment maintenance, supply inventory, and basic administrative tasks.
Professional Responsibilities
- Maintain certifications and participate in ongoing training.
- Participate in on-call rotation as assigned and support organizational mission and values.

Skills, Knowledge & Expertise
- Current, active, and unencumbered Practical Nurse (LPN) license in the State of Michigan, required.
- Valid Michigan driver’s license and reliable transportation to conduct home visits.
- Minimum of 1 year of experience working with a frail or elderly population, required.
- 3 years of LPN experience in a geriatric setting, preferred.
- Ability to lift up to 35 pounds independently to support participants as needed.
- CPR certification through the American Heart Association (AHA), or willingness to obtain it through LifeCircles.
